Purpose of the Position: The Inner-City Family Health Team is a multidisciplinary team that serves homeless and previously homeless patients with complex health needs. These include untreated chronic and complex multi-system diseases, an elevated risk of acute health emergencies, and serious and persistent mental health and substance use challenges. Services are provided by the team using a holistic, patient-centred model of care with an emphasis on disease prevention, chronic disease management, harm reduction and health promotion. The successful candidate will predominantly work at our downtown clinic and provide mobile clinics to community partners. ICFHT is looking for a physiotherapist with the knowledge, skills, and competencies to support individuals and populations across the continuum of care, including health promotion and illness prevention, self-management, treatment, and rehabilitation. The physiotherapist will assess the impact of injury, disease or disorders on the patient’s mobility and function. Physiotherapy care will encompass promoting, restoring, and prolonging physical independence by maintaining and/or enhancing a patient’s functional capacity. We are seeking someone with the compassion and skill to work with marginalized patient populations, many of whom have complex physical health issues, mental health, active substance uses and cognitive impairment. Reporting to the Clinical Manager, the physiotherapist collaborates with the Health Team’s Family Physicians, Psychiatrists, our interdisciplinary team, and community/shelter staff to provide timely, effective, and appropriate physiotherapy in accordance with FHT policies and professional standards.
