Purpose of the Position:

The Inner-City Family Health Team is a multidisciplinary team that serves homeless and previously homeless patients with complex health needs. These include untreated chronic and complex multi-system diseases, an elevated risk of acute health emergencies, and serious and persistent mental health and substance use challenges. Services are provided by the team using a holistic, patient-centred model of care with an emphasis on disease prevention, chronic disease management, harm reduction and health promotion. The successful candidate will predominantly work at our downtown clinic and provide mobile clinics to community partners.

ICFHT is looking for a physiotherapist with the knowledge, skills, and competencies to support individuals and populations across the continuum of care, including health promotion and illness prevention, self-management, treatment, and rehabilitation. The physiotherapist will assess the impact of injury, disease or disorders on the patient’s mobility and function. Physiotherapy care will encompass promoting, restoring, and prolonging physical independence by maintaining and/or enhancing a patient’s functional capacity. We are seeking someone with the compassion and skill to work with marginalized patient populations, many of whom have complex physical health issues, mental health, active substance uses and cognitive impairment.

Reporting to the Clinical Manager, the physiotherapist collaborates with the Health Team’s Family Physicians, Psychiatrists, our interdisciplinary team, and community/shelter staff to provide timely, effective, and appropriate physiotherapy in accordance with FHT policies and professional standards.

Key Responsibilities:

  • Perform subjective and objective assessments to identify the impact of injury, disease or disorders of movement and function and patient goals.
  • Implement a treatment plan, coordinating with interdisciplinary providers to promote, restore, and prolong physical independence by enhancing a patient’s functional capacity.
  • Physiotherapists will assess and treat individuals across the lifespan (non-pediatric in this FHT context) who have illness, injury or disability affecting the neuro-musculoskeletal, cardiopulmonary, vascular, and neurological systems; they also promote wellness and increased quality of life through education, specialized programming, and a holistic approach to care.
  • Use evidence-based/best practice guidelines in developing appropriate physiotherapy care plans as well as in developing policies and procedures aligned with the organization’s strategic direction.
  • Provide services in the clinic setting and in community mobile clinics and partnering shelters.
  • Document all aspects of the therapeutic processes in the patient’s medical record.
  • Physiotherapists will have the knowledge, skills, and competencies to support individuals and populations across the continuum of care, including health promotion and illness prevention, self-management, treatment, and rehabilitation.

Experience, Credentials, Skills, and Qualifications

  • Minimum one year of related clinical experience preferred.
  • Must demonstrate ability to conduct assessments, identify and implement care plans, document health records, and communicate and collaborate with an interdisciplinary care team.
  • Other key competencies include the ability to work in an interdisciplinary team environment, the ability to prioritize, manage time effectively and be flexible in a very active work environment, and compliance with practice standards from the College of Physiotherapists of Ontario.
  • Knowledge of and commitment to client–centred philosophy and basic principles and practice of chronic disease management models.
  • Open-minded, non-judgmental with a commitment to upholding the rights and dignity of all people and respecting their diversity.
  • A sound understanding of and commitment to the Social Determinants of Health and the ability to effectively participate in the efforts of an interdisciplinary team to contribute to comprehensive health care.
  • Experience working with homeless or marginalized services is considered an asset.
  • Qualified Assistive Device Program Authorizer is considered an asset.
